Body compressed; anterior and posterior nostrils tubular; gill membranes broadly joined to body under throat, with 5 rays; dorsal fin VII (none filamentous) + I, 11; length of 2nd dorsal base > distance from 2nd dorsal base to tail fin; anal fin I, 11; pelvic fins I, 5, completely separate, rays 1-4 branched 2-4 times, without flattened, fleshy tips, 4th reaching to anus, 5th ray not branched, ¾ length of 4th, 4th ray reaching ¾ distance to anus; tail fin oval; scales mainly rough, cover from beneath pectoral fin to tail fin base, in lateral series 47-53; with large rough scales on base of tail fin; no scales on center and front of abdomen; no scales from rear end of 1st dorsal base to upper pectoral base; no lateral line.
Head and body ground color white; 4 large oval yellow-brown, dark-centered blotches along center of flank from under 1st dorsal to end of tail base; 3 short, paler yellow-brown double bars across upper midline, 1 under 1st dorsal, 2 under 2nd dorsal, those positioned between flank blotches; a yellow saddle on upper tail base; 3 yellow bars across nape, the front 2 curving forwards on the side of the head; yellow spots below eye and on preoperculum; iris yellow; dorsal and tail fins white, anal fin and pelvics with grey base, black edges
Reaches at least 6.7 cm
Lives on sand and rubble.
Depth range 282 m
Known from a single specimen taken in the Bahamas.
