Head large, blunt, front profile steep and rounded; body elongate, compressed; nostrils with a flap bearing 1-3 cirri; a single cirrus over eye, main shaft rounded, with 1-6 simple branches; mouth opens at front; top margin of top lip free, that margin discontinous at front; 3-4 pores along lower jaw; jaw teeth are brush-like, with a large curved canine at rear of each jaw; gill membranes broadly fused on throat, gill opening ends at level of lower base of pectoral fin; dorsal fin XII, 12-14, with a distinct notch between spiny and soft parts; anal fin II, 15; pelvics I, 4; pectorals 13-14 rays; fin rays unbranched, except on tail fin; lateral line in 2 parts, 1st curved down behind end of pectoral, ends under last dorsal spines, 2nd from mid-flank to above ~middle of anal fin; no scales.
Grey-brown above, paler below; head red-brown, with orange spots that may continue onto body; body white, with 6 bars that are thicker and darker above (black to dark brown) above, and thinner and less strongly colored below; tail fin tan, with an elongate dark spot on its base and 3-5 dark bars; dorsal and anal fins lightly barred.
Reaches 12 cm.
Lives on a variety of rocky habitats, rubble, and coral reefs.
Depth range: 1-27 m.
Bermuda to the SW Gulf of Mexico.
